Our new patient fees usually range from $38.00 - $51.00, and our established patient prices customarily run $25.00 - $38.00 per visit.   Flexible payment plan arrangements can be made if you are interested.

OUR FINANCIAL POLICY IS DESCRIBED BELOW

HEALTH PLANS:   We are members of Vantage Healthcare Network, First Health, Intergroup, and Multi-Plan.  If you belong to these plans, please let us know.  Each plan has specific guidelines regarding care and payment.  We are not members of Health America or UPMC Health Plan.  However, with respect to UPMC Health Plan, if your card shows the Vantage logo, there may be coverage available for our services.

HIGHMARK BLUE CROSS/BLUE SHIELD:   We are a participating Highmark provider.  Highmark has a number of plans with specific guidelines and varied co-payments.  We will make a copy of your card in order to investigate those guidelines.

WORKERS' COMPENSATION:   If your injury occurred at work, your charges may be covered under Workers' Compensation. The procedure to follow will be outlined for you upon completion of the Patient History Form.

AUTO INJURIES:   If your injury occurred as a result of an auto accident, the procedures to follow will be outlined for you upon completion of the Patient History Form.

MEDICARE:   We do participate in the Medicare Program.  Further information about the detailed policies regarding Medicare will be given to you upon completion of the Patient History Form.  

MEDICAID:   We do not accept reimbursement from the Medical Assistance Program.
